Job description
Altogether Care are looking for a Care Coordinator to work alongside our community care team.
You will need a car for work purposes for this role.
What hours am I expected to work?
40 hours per week – Evenings will be required when needed to meet the demands of the service.
Weekend Working
Every other weekend

Organising staff rotasBuilding relationships with care staff and clients.Providing personal care and support within our clients homes in the local communitySupporting the on–call service on a rota basis What do YOU need?
At least 12–18 months care experienceA can–do attitude, caring and reliable natureExcellent communicationBe able to develop and maintain good working relationships with colleagues, clients and their families Required Criteria
